What is the purpose?

Why should I / we do it?

1) It has been consistently reported that use of THE CARE PLAN with each hospice admission will:

  • Increase staff efficiency while it improves the quality of symptom management.

  • Decrease triage and after-hour calls. 

  • Increase CAHPS scores (Consumer Assessment of Healthcare Providers & Systems Hospice Survey).

What do I / We have to do?

STEP 1:  Register HERE to receive your 10 free copies.

STEP 2: Select the RN Case Managers - We suggest two RNs who will utilize the booklet with 5 new hospice admissions.

STEP 3: Once the booklets are received, each selected RN case manager views the Tips for Use or Video Webinar.

STEP 4: The RN Case Manager introduces the booklet during the admission process and uses it over 4 weeks.

STEP 5: At the conclusion of the 4 weeks, the RN fills out the online survey for each patient.

STEP 6: Accept our heartfelt gratitude for taking your valuable time to evaluate The Care Plan as we all work to improve hospice care.
